În acest context, participarea prof.dr. Petrișor Geavlete, șeful clinicii de urologie

Spital clinic de urgență Sfântul Ioan București, cu redactarea unui capitol de bază

reprezintă un moment cu foarte puține echivalențe, la acest nivel, indiferent de

specialitatea unui medic român. De fapt, invitația de a redacta un astfel de capitol se

bazează pe cele peste 1.000 de contribuții științifice la manifestări urologice naționale

și internaționale, articole (47 în reviste cotate și indexate ISI) și abstracte, prezentări

podium session, postere și video, etc. În special în endourologie, profesorul

P.Geavlete are o experiență și contribuții semnificative unanim recunoscute.

Revenind la Tratatul de Endourologie, redactat sub auspiciile unor extrem de reputați

urologi americani, trebuie menționat în mod special profesorul, Arthur

Smith, „părintele endourologiei mondiale”. Trebuie amintit faptul că „Smith's

Textbook of Endourology is the definitive reference book in the field, addressing

every aspect of endourologic procedure including methods of access, operative

techniques, complications, and postoperative care”, așa cum apare pe pagina de

prezentare a tuturor mijloacelor electronice de prezentare.

In 1807, Charles Wiley, then 25 years old, opened a small printing shop at 6 Reade

Street in lower Manhattan, New York City. During the next four years, he worked

with other printers, primarily Isaac Riley, printing and publishing law books. In 1812,

"C. Wiley, Printer" appeared for the first time on the title pages of several legal

works.

When Charles Wiley died in 1826, his son John, then age 18, took over the family

business. Over the next 65 years, he continued as a bookseller and publisher,

promoting American and European authors. In 1836, he hired George Putnam as a

junior partner; together they achieved prominence publishing such works as Herman

Melville's Typee: A Peep at Polynesian Life, Edgar Allan Poe's The Raven, and

and Elizabeth Barrett Browning.

In 1956, the year before the company celebrated its 150th anniversary, W. Bradford

Wiley, then 46, succeeded his cousin as President and Chief Executive Officer. The

following years were a time of significant expansion and change.

Those years also brought another surge in international growth. Subsidiaries were

established in London in 1959 (moving to Chichester in 1968); Australia in 1963;

India in 1965; and Toronto, Canada in 1968. In subsequent years, Wiley expanded its

global operations to Singapore, Japan, and Germany, and set up sales offices on every

continent.

Wiley established www.wiley.com as its corporate Web site in May 1995. The site

has evolved considerably since then, and now offers the full range of Wiley's

products, brands, and services. Wiley InterScience launched commercially in January

1999 as the company's online platform for scientific, technical, medical, and

professional content. The site was replaced in August 2010 by Wiley Online Library

(wileyonlinelibrary.com, described below).

In 2007 Wiley completed the acquisition of the outstanding shares of the U.K.-based

Blackwell Publishing (Holdings) Ltd., one of the world's foremost academic and

professional publishers. Blackwell's publishing program has been merged with

Wiley's global scientific, technical, and medical business to form what is now called

Scientific, Technical, Medical, and Scholarly, also known as Wiley-Blackwell, the

În general vorbind, analizarea unor posibile complicații legate de o anumită

procedură chirurgicală este mai puțin atractivă. Vă rog să descrieți relația dintre

chirurgia minim invazivă, a ureteroscopiei în special și complicațiile specifice.

Orice pacient, atunci când dorește să fie tratat cât mai puțin traumatic, dacă se poate

„cu laserul” (expresie relativ frecventă a multor bolnavi), alege această alternativă în

ideea unei operații mai ușor de suportat, cu reintegrare socială rapidă, fără cicatrici

sau toate neajunsurile date de operația clasică. Trebuie să spun că eu am fost crescut

de profesorii mei în spiritul chirurgiei clasice! O consider încă o alternativă de mare

valoare pentru mulți bolnavi. Pentru noua generație de specialiști urologi, ea trebuie să

fie cunoscută cât mai bine posibil. În ciuda mirajului chirurgiei cu monitoare HD, a

instrumentelor flexibile digitale, a tehnicilor în fluorescență, a utilizării mediului salin,

a chirurgiei laparoscopice robotice, a evoluției către tridimensional, etc., etc., consider

că încă nu a venit momentul să considerăm chirurgia clasică arhaică, depășită, total

uitată pentru o multitudine de afecțiuni urologice! Pentru că am vorbit de posibile

complicații, multe secții și clinici occidentale au încă norocul unor specialiști care știu

chirurgie clasică! Nu știu ce se va întâmpla într-un viitor nu prea îndepărtat!

Și acum revenind la întrebarea d-voastră, după o experiență consider semnificativă,

pot spune că uneori este greu să definești și să susții în toate cazurile așa zisa minim

invazivitate a procedurilor endoscopice. Este adevărat că efectuate corect, cu o tehnică

strictă și apelând la măsurile de asigurare a siguranței, tehnicile endoscopice au puține

evenimente nedorite. În marea majoritate a cazurilor, chirurgii prezintă reușitele,

operațiile deosebite, rezultatele foarte bune, curajul și tehnicitatea, anvergura

operațiilor și diversitatea lor. Cred cu convingere că este corect să fie analizate și

complicațiile procedurilor. Și mai ales în cazul utilizării endoscopiei și laparoscopiei,

alternative de la care se așteaptă de fapt cel mai mult din partea bolnavului!

Complicația posibilă trebuie să fie în primul rând prevenită prin acuratețea abordării,

prin abilitate și răbdare, prin luarea tuturor măsurilor de siguranță. Atunci când totuși

apare ea trebuie recunoscută la timp și tratată adecvat. Oprirea procedurii și drenajul

pot salva o intervenție care devine invazivă! În această idee, consider că apariția unui

Tratat care să conțină și capitole cu abordarea unor astfel de probleme este extrem de

importantă pentru un specialist care se confruntă cu astfel de situații, aș spune din

toate punctele de vedere. În final, aș dori să subliniez faptul că prezența printre autorii

Tratatului Smiths Textbook of Endourology reprezintă pentru mine o extrem de

onorantă poziție, reliefând atât experiența personală recunoscută la acest nivel, cât și

activitatea întregii clinici de urologie de la Spitalul clinic de urgență Sfântul Ioan

București. Și, nu în ultimul rând, sunt extrem de bucuros pentru că fac parte dintr-o

școală de medicină cu tradiție și cu recunoaștere internațională, la care contribuie și

alți specialiști de marcă din diverse zone medicale.

Una dintre realizările editoriale internaționale de excepție este reprezentată de editarea

a 3 capitole într-un Tratat internațional al European Association of Urology (EAU)

Urolithiasis Society (EULIS) într-o editura italiană de referință. Este o nouă

recunoaștere, la cel mai înalt nivel, a experienței, expertizei și a prezenței internaționale

a unui urolog român.
